Insurance Coverage For Silicone Breast Forms

All silicone breast forms are covered by Pharmacare and most private insurances. Pharmacare covers a maximum of $450 for a Mastectomy, and $350 for a Lumpectomy (for a partial prosthesis), every 2 years.

Pharmacare does not cover Mastectomy bras but some private insurances do. Most women with extended benefits can get 1 or 2 bras per year depending upon their coverage. A letter of diagnosis from your doctor and a receipt of the prosthesis/bra is all that is needed.
